It intends to do this by facilitating the earlier discharge of hospitalized patients and by providing Out of Hospital (preferably home based) solutions for the diagnosis, treatment and management of patients that obviate the need for hospitalization.

Sleep apnea is effectively a collapse of the airways that occurs while sleeping causing suffocation. An event is if breathing is halted for more than 10 seconds. The body’s natural response is to wake itself through release of adrenalin, increased pulse rate, higher blood pressure and respiratory rate. Once the patient gasps air, they will fall back to sleep- in most cases not even aware such an event has occurred.

Severe OSA is greater than 30 events per hour resulting in life reduction (6 years on average) and cardiac events (within 10 years on average).

Linked to achieving the objectives “hospital to home” and home care is the necessity of having medical grade vital signs monitoring on patients.

This is also termed as “wearables”. These devices need to communicate to call centers to ensure patients are managed effectively and in a 24/7 approach.

  • Post Acute management: early release from hospital
  • Psychiatric management: initial drug regime management
  • Frail and Retirement Care: prevent unnecessary hospital admittance

This focus is to drive a more digital, clinically integrated, and cloud-based approach for Out of Hospital solutions, whilst ensuring the highest level of clinical oversight and adherence to treatment regimens.

This will ensure the earlier discharge of patients from hospital and will allow appropriate recall of the patient to their physician, based on monitored clinical changes to the patient’s condition.
